TITLE:    PERSONAL INDEPENDENCE SERVICES CASEMANAGER

RESPONSIBLE TO:     PERSONAL INDEPENDENCE SERVICES PROGRAM DIRECTOR

 

OBJECTIVE:   UNDER THE DIRECTION OF THE PERSONAL INDEPENDENCE SERVICES PROGRAM

DIRECTOR; DEVELOP, IMPLEMENT AND MONITOR INDIVIDUAL PLANS; MAINTAIN

COMMUNICATION WITH PARTICIPANTS AND THEIR PARENTS/GUARDIANS OR INTERESTED

THIRD PARTIES; MAINTAIN AN INTEGRATED AND SEMI-COMPREHENSIVE PROGRAM

THROUGH COORDINATION WITH PERSONAL INDEPENDENCE SERVICES STAFF.

 

DUTIES:

*           ACTS AS DIRECT SUPERVISOR FOR INSTRUCTORS AS ASSIGNED.

*           MAINTAINS ON-GOING COMMUNICATION WITH PERSONAL INDEPENDENCE

PROGRAM DIRECTOR AND OTHER PERSONAL INDEPENDENCE CASEMANAGER.

*           ACTS AS SUPERVISOR DURING ABSENCE OF PROGRAM DIRECTOR.

*           COORDINATES TRIPS OUT IN COMMUNITY AND TRANSPORTATION.

*           PROVIDES DAY-TO-DAY GUIDANCE TO STAFF, AS ASSIGNED.

*           REQUESTS MAINTENANCE REPORTS AND JANITORIAL SERVICES NEEDED.

*           ATTENDS SPECIAL STAFFINGS AND RECORDS MINUTES.

*           CONVENES ANNUAL PLANNING MEETINGS.

*           MONITORS GOAL PROGRESS ON ALL PARTICIPANTS.

*           PREPARES DISCHARGE SUMMARIES AND FOLLOW-UP REPORTS.

*           MAINTAINS INTER-DEPARTMENTAL COMMUNICATION.

*           ATTENDS WEEKLY STAFF MEETINGS.

*           COMPLETES PERFORMANCE REVIEWS FOR ASSIGNED INSTRUCTORS.

*           PROVIDES INPUT ON STAFF PERFORMANCE REVIEWS.

*           MONITORS PAPERWORK FROM INSTRUCTORS.

*           SERVES AS A ROLE MODEL FOR STAFF AND PARTICIPANTS.

*           MAINTAIN LISTING OF EDUCATIONAL MATERIALS.

*           MONITORS SCHEDULE CHANGES DUE TO STAFF ABSENCES.

*           MAINTAINS KNOWLEDGE OF CURRENT TRENDS IN INTELLECTUAL/DEVELOPMENTAL

DISABILITIES, CARF STANDARDS, ICF/ID AND HCBS REGULATIONS.

*           ATTENDS AND HELPS PLAN IN-SERVICE TRAINING SESSIONS.

*           MONITORS CLASS ACTIVITIES.

*           PROVIDES DIRECT CARE FOR PARTICIPANTS AS NEEDED.

*           ADHERES TO THE CENTERS CALL IN AND TIME OFF POLICY.

*           MAINTAIN EXCELLENT ATTENDANCE AND PUNCUALITY TO ENSURE ADEQUATE

STAFFING LEVELS TO MEET THE NEEDS OF PERSONAL INDEPENDENCE SERVICES PARTICIPANTS.

*     INFORM SUPERVISOR AT LEAST TWO WEEKS PRIOR TO USING VACATION OR PERSONAL

TIME WHENEVER POSSIBLE.

OTHER DUTIES AS ASSIGNED.

 

QUALIFICATIONS:

 

KNOWLEDGE AND ABILITIES:

KNOWLEDGE OF THE PRINCIPLES AND TECHNIQUES INVOLVED IN SERVICES TO

THE DEVELOPMENTALLY DISABLED.

ABILITY TO COMMUNICATE ARTICULATELY BOTH ORALLY AND WRITTEN.

ABILITY TO ACTIVELY ENCOURAGE STAFF MEMBERS TO WORK TOGETHER.

ABILITY TO MEET THE PARENTS/GUARDIANS AND OUTSIDE AGENCIES IN A

KNOWLEDGEABLE MANNER.

ABILITY TO RECOGNIZE PROGRAM PROBLEM AREAS AND TO INITIATE THE

CORRECTION OF SUCH.

TRAINING AND EXPERIENCE:

BACHELOR’S DEGREE REQUIRED

MAJOR IN A HUMAN SERVICES FIELD PREFERRED

MINIMUM OF TWO YEARS OF WORK RELATED EXPERIENCE.

Full-time professional position is responsible for coordinating and monitoring services to assigned CRS participants.  This includes development of service plans, implementation of services and necessary documentation and paperwork. Duties also include scheduling and supervision of staff and rotating on-call responsibility.
 
Bachelor’s degree in Human Services required; experience working in ID/DD setting preferred. Ability to use basic computer programs (Microsoft Office suite) to complete electronic documentation and basic computer functions.

RESPONSIBLE TO:     COMMUNITY RESIDENTIAL SERVICES PROGRAM DIRECTOR OR ASSISTANT
                                    PROGRAM DIRECTOR

 
            OBJECTIVE:   COORDINATE AND MONITOR SERVICES TO ASSIGNED CRS PARTICIPANTS
INCLUDING DEVELOPMENT OF SERVICE PLAN, IMPLEMENTATION OF SERVICES AND
NECESSARY DOCUMENTATION.  SUPERVISE ASSIGNED SERVICE STAFF.
 
            DUTIES:
  • WITH OTHER RESIDENTIAL STAFF AND INDIVIDUALS, EVALUATE EACH INDIVIDUAL'S FUNCTIONING AND ASSESS HIS/HER SERVICE NEEDS.
  • WORK WITH DIRECT SERVICES STAFF AND PARTICIPANT TO FORMULATE GOALS AND OBJECTIVES FOR EACH PERSON, TO ACCURATELY DOCUMENT AND ASSESS PROGRESS TOWARD SUCH GOALS AND TO ASSESS INDIVIDUAL NEEDS OF PARTICIPANTS.
  • PROVIDE APPROPRIATE CASEWORK AND COUNSELING SERVICES TO INDIVIDUALS, INDIVIDUALLY AND/OR IN SMALL GROUPS, AS APPROPRIATE.
  • MAINTAIN ACCURATE COMMUNICATION WITH FAMILIES OF INDIVIDUALS, AND PROVIDE COUNSELING AS APPROPRIATE.
  • MAINTAIN ACCURATE PROFESSIONAL RECORDS ON EACH INDIVIDUAL TO DOCUMENT PLANNING, PROGRESS AND REASSESSMENT BY RESIDENTIAL STAFF, AS WELL AS TO RECORD ESSENTIAL DATA REGARDING INDIVIDUAL’S ACTIVITIES, MEDICAL
    NEEDS AND TREATMENT, AND OTHER SIGNIFICANT EVENTS.
  • DEVELOP AND MAINTAIN GOOD CHANNELS OF COMMUNICATION AMONG RESIDENTIAL STAFF, AND BETWEEN RESIDENTIAL STAFF AND OTHER HDC PROGRAM PERSONNEL INVOLVED WITH PARTICIPANTS. THE GOAL IS COORDINATED AND CONSISTENT PLANNING AND PROGRAMMING FOR EACH INDIVIDUAL.
  • PROVIDE INFORMATION TO INDIVIDUALS, GROUPS, AGENCIES, AND PARTICIPANTS REGARDING THE CENTER’S PROGRAMS.
  • ACQUIRE WORKING KNOWLEDGE OF LAWS, REGULATIONS, AND POLICY UNDER WHICH PROGRAM IS OPERATED AS REQUIRED TO DISCUSS SUCH MATTERS WITH PARTICIPANTS, FAMILIES, AND OTHER AGENCIES AND TO ENSURE COMPLIANCE WITH THE INDIVIDUAL PROGRAMS.
  • ASSIST COMMUNITY RESIDENTIAL SERVICES PROGRAM DIRECTOR IN PLANNING AND DEVELOPING COMMUNITY RESIDENTIAL SERVICES PROGRAM.
  • PROVIDE TRAINING AND DAY-TO-DAY GUIDANCE OF DIRECT SERVICE STAFF.
  • ASSIST IN TRAINING AND SUPERVISION OF VOLUNTEERS WORKING WITH PARTICIPANTS.
  • RELATE TO OTHER SOCIAL SERVICE, HEALTH, AND EDUCATIONAL AGENCIES IN THE COMMUNITY ON BEHALF OF PARTICIPANTS.
  • SUPERVISE AND COORDINATE THE WORK OF ASSIGNED RESIDENTIAL STAFF.
  • RELATE CONTACT WITH OUTSIDE AGENCIES/INDIVIDUALS IMMEDIATELY TO SUPERVISOR.
  • ACT AS A MEDICATION MANAGER AS NEEDED.
  • MAINTAIN EXCELLENT ATTENDANCE AND PUNCTUALITY TO ENSURE PARTICIPANT NEEDS ARE MET.  REGULAR AND ON SITE ATTENDANCE REQUIRED.
    • RESPONSIBLE FOR ON-CALL DUTIES.
      OTHER DUTIES AS ASSIGNED.

                                          CARRIER.
                              NO CURRENT RECORD OF HABITUAL ALCOHOL INTOXICATION OR ADDICTION TO THE
                                          USE OF DRUGS.
                              ABILITY TO CARRY/ESCORT INDIVIDUALS IN EXCESS OF 50 POUNDS AND UTILIZE CPI
                                          TECHNIQUES DURING BEHAVIOR PROBLEMS.
       
                  TRAINING AND EXPERIENCE:
                              BACHELOR’S DEGREE IN HUMAN SERVICES, SOCIAL WORK, EDUCATION OR A RELATED
      FIELD AND A  MINIMUM OF ONE YEAR EXPERIENCE IN SOCIAL SERVICES. 
                              EXPERIENCE IN A SETTING WITH PEOPLE WITH INTELLECTUAL DISABILITIES IS
                                          PREFERRED.
