Description

Under the direction of the Program Director, this full-time position is responsible for residential and programmatic aspects of one ICF/ID unit. Includes planning, organizing, coordination and evaluating in the areas of safety, staffing, resource utilization, program development and implementation and coordination of all services. Must have Bachelor’s degree in Human Services and qualify as QIDP; previous experience in field of ID/DD required. Supervisory experience preferred.

RESPONSIBLE TO:     PROGRAM DIRECTOR

 

KIND OF WORK:

 

                OBJECTIVE: UNDER THE DIRECTION OF THE PROGRAM DIRECTOR, RESPONSIBLE FOR RESIDENTIAL AND PROGRAMMATIC ASPECTS OF ONE ICF/ID UNIT.  INCLUDING PLANNING, ORGANIZING, COORDINATING AND EVALUATING IN THE AREAS OF SAFETY, STAFFING, RESOURCE UTILIZATION, PROGRAM DEVELOPMENT AND IMPLEMENTATION AND COORDINATION OF ALL SERVICES.

 

              DUTIES:

                            *DIRECTLY SUPERVISES ALL ICF/ID DIRECT CARE  STAFF (PROGRAM LEADS AND

                                      RESIDENT AIDES, RECREATION SPECIALIST, PT/OT AIDE, SPEECH THERAPY

                                      ASSISTANT ON THE DESIGNATED WING).

                            *DIRECTLY SUPERVISES SHIFT SUPERVISORS.

                                      ASSISTANT ON THE DESIGNATED WING).

                            *PARTICIPATES IN UNEMPLOYMENT HEARINGS.

                            *SERVES ON COMMITTEES AS DIRECTED BY SUPERVISOR.

                            *IDENTIFIES STAFFING NEEDS AND SCHEDULE STAFF TO MAINTAIN COVERAGE                                                               ACCORDING TO APPLICABLE REGULATIONS AND STANDARDS, APPROVE LEAVE                                               REQUESTS AS APPROPRIATE.

                            *ASSISTS WITH HIRING PROCESS AS REQUESTED.

                            *CONDUCTS DISCIPLINARY ACTION AS NEEDED.

                            *COMPLETES PERFORMANCE APPRAISALS FOR DESIGNATED DIRECT CARE STAFF.

                            *ENSURES COMPLIANCE WITH REGULATIONS GOVERNING PHYSICAL FACILITY                                                              MANAGEMENT OF ICF/ID UNITS.

                            *COORDINATES ON-GOING COMPREHENSIVE BEHAVIOR MANAGEMENT,

                                      COMMUNICATION, AND SELF-HELP SKILLS TRAINING FOR INDIVIDUALS

                                      ACCORDING TO ICF/ID GUIDELINES.

                            *TRAINS SHIFT SUPERVISORS AND DIRECT CARE STAFF IN SELF-HELP SKILLS, PROGRAM                                            IMPLEMENTATION, AND DOCUMENTATION.

                            *MONITORS INTEGRATION OF HDC PROGRAMS WITH THOSE OF SCHOOLS AND DAY                                                       PROGRAMS.

                            *ENSURES MCO CASEMANAGERS RECEIVE UPDATES AS REQUIRED.

                            *IN CONJUNCTION WITH RECREATION STAFF, ENSURES LEISURE ACTIVITIES AND                                                          TRAINING PROGRAMS MEET NEEDS OF INDIVIDUALS.

                            *ASSISTS RECREATION STAFF IN TRAINING DIRECT CARE STAFF TO ASSIST WITH

                                      AND/OR INDEPENDENTLY IMPLEMENT LEISURE ACTIVITIES AND TRAINING

                                      PROGRAMS.

                            *ENSURES THAT AN ACTIVE PROGRAM OF TREATMENT IS PROVIDED TO EACH

                                      INDIVIDUAL.

                            *INTERPRETS AND ENFORCES AGENCY POLICIES AND PROCEDURES; SUGGEST CHANGES                                            TO PROGRAM DIRECTOR.

                            *COMMUNICATES WITH ALL ICF/ID STAFF.

                            *ATTENDS ALL REQUIRED MEETINGS AND TRAININGS.

                            *SERVES AS MEMBER OF ID TEAM FOR INDIVIDUALS LIVING ON ICF/ID UNITS.

                            *PROVIDES WRITTEN REPORTS AND STATISTICAL SUMMARIES.

                            *ENSURES ALL IPP DOCUMENTATION IS MAINTAINED FOR EACH INDIVIDUAL.

                            *MAINTAINS ANNUAL SCHEDULE OF STAFFINGS ON UNIT.

                            *HELPS FACILITATE STAFFINGS.

                            *PROVIDES DIRECT CARE WHEN NEEDED.

                            *ENSURES THAT ALL FEDERAL AND STATE REGULATIONS ARE MET.

                            *ENSURES COMPLIANCE WITH MCO EXPECTATIONS.

                            *ENSURES THE GENERAL HEALTH, SAFETY, AND PRIVACY OF INDIVIDUALS.

                            *REVIEWS WING BOOKS AS NECESSARY TO ENSURE ACCURACY OF CONTENTS.

                            *ENSURES A HIGH QUALITY OF LIFE FOR EACH INDIVIDUAL.

                            *ENSURES ALL PROFESSIONAL RECOMMENDATION ARE REVIEWED AND IMPLEMENTED.

                            *INTERACTS WITH SURVEY TEAMS AND DEVELOPS PLANS OF CORRECTION REGARDING                                             DEFICIENCIES.

                            *COORDINATES TRANSPORTATION TO SCHOOL/DAY PROGRAM AS NEEDED.

                            *MAINTAIN CONFIDENTIALITY OF ALL INFORMATION TO ASSURE RIGHTS ARE                                                PROTECTED.

                            *FOLLOWS ALL HDC POLICIES AND PROCEDURES.

                            *PROMOTES THE MORALE OF ALL PERSONNEL AND MAINTAINS A SPIRIT OF

                                      COOPERATION. 

                            *MAINTAINS EXCELLENT ATTENDANCE AND PUNCTUALITY.

                            *ADHERES TO THE CENTER’S CALL IN AND TIME OFF POLICY.

                            *UPHOLDS THE MISSION AND PHILOSOPHY OF THE CENTER.

                            *ENSURES ALL RESIDENTS ARE TREATED WITH DIGNITY AND RESPECT.

                            *PROVIDES NECESSARY SUPPORTS TO ALL RESIDENTS REGARDLESS OF HEALTH OR

                                      BEHAVIORAL CONCERNS.

QUALIFICATIONS:

 

              KNOWLEDGE AND ABILITIES:

                            KNOWLEDGE OF HUMAN DEVELOPMENT, DEVELOPMENTAL DISABILITIES AND MENTAL                                             RETARDATION.

                            KNOWLEDGE OF SUPERVISORY PRACTICES AND METHODS.

                            UNDERSTANDING OF PROGRAMMATIC VS.  MEDICAL SERVICE MODEL.

                            KNOWLEDGE OF COMMUNITY, SOCIAL EDUCATION AND HEALTH AGENCIES AND THE                                  SERVICES THEY PROVIDE.

                            ABILITY TO EXPRESS ONESELF BOTH VERBALLY AND IN WRITTEN FORM.

                            ABILITY TO LIFT FROM FLOOR/CARRY/TRANSFER INDIVIDUALS AND ADAPTIVE                                                              EQUIPMENT IN EXCESS OF 50 POUNDS.

                            ABILITY TO CARRY/ESCORT INDIVIDUALS IN EXCESS OF 50 POUNDS AND UTILIZE

              TRAINING AND EXPERIENCE:

                            BACHELOR’S DEGREE IN APPROVED FIELD TO QUALIFY AS QIDP.

                            MINIMUM OF TWO YEARS EXPERIENCE IN PROGRAM DEVELOPMENT AND INSTRUCTION                                            WITH ID/DD.

                            SUPERVISORY EXPERIENCE PREFERRED.
