EMPLOYEE JOB DESCRIPTION
TITLE: PT/OT AIDE DEPARTMENT: ICF/ID
RESPONSIBLE TO: PROGRAM SUPERVISOR
KIND OF WORK:
OBJECTIVE: UNDER THE DIRECTION OF THE SHIFT SUPERVISOR AND THE SUPERVISION OF THE PROGRAM SUPERVISOR, PROVIDE DAY TO DAY CARE OF INDIVIDUALS SERVED AND ASSIST IN THE DEVELOPMENT AND MAINTENANCE OF SELF-HELP SKILLS IN THOSE INDIVIDUALS. ADDITIONALLY, IN COOPERATION WITH THE PHYSICAL AND OCCUPATIONAL THERAPY CONSULTANTS, PLAN, PREPARE, IMPLEMENT AND MONITOR INDIVIDUALS’ PROGRAMS AND ACTIVITIES.
DUTIES:
*COMMUNICATES WITH THE PT AND OT CONSULTANTS AND CARRIES OUT RECOMMENDATIONS PROMPTLY AND ACCURATELY.
*PLANS, PREPARES, IMPLEMENTS AND DOCUMENTS PT/OT PROGRAMS INCLUDING CONDUCTING BASELINES.
*COMPLETES PROGRAM DATA WEEKLY AND MAKES REVISIONS AS NEEDED AFTER DISCUSSION WITH THE PROGRAM SUPERVISOR AND/OR PT/OT CONSULTANT.
*DOCUMENTS PROGRESS, CONCERNS, OR INDIVIDUAL NEEDS AND COMPLETES MONTHLY REPORTS FOR EACH INDIVIDUAL ON CASELOAD.
*MAINTAINS ON-GOING COMMUNICATION AND COORDINATES ALL PROGRAMS WITH DISCIPLINES INVOLVED WITH THE INDIVIDUALS’ PLAN OF CARE.
*MONITORS PROGRAM BOOKS TO ENSURE ACCURATE DOCUMENTATION OF PROGRAMS, APPROPRIATE DAILY LOGGING, AND CURRENT PROGRAMS AND DATA SHEETS.
*NOTIFIES PROGRAM SUPERVISOR OF NEEDED TRAININGS AND CONDUCTS TRAININGS
AS NEEDED.
*ENSURES NEEDED SUPPLIES/EQUIPMENT ARE AVAILABLE AND IN WORKING ORDER.
*PLANS, IMPLEMENTS AND MONITORS DAILY ACTIVITY SCHEDULE.
*COMPLETES REQUESTS FOR ACTIVITIES AND NECESSARY ITEMS IN COOPERATION
WITH THE RECREATION DEPARTMENT.
*SUBMITS COMPLETED IPP PACKET TO PROGRAM SUPERVISOR IN ACCORDANCE WITH ESTABLISHED PROCEDURES.
*ATTENDS AND PARTICIPATES IN STAFFINGS.
*PERFORMS DIRECT CARE FOR ASSIGNED INDIVIDUALS SO THAT PRESCRIBED CARE IS
MAINTAINED.
*COMPLETES ASSIGNED NURSING DUTIES.
*KEEPS INDIVIDUALS CLEAN AND APPROPRIATELY DRESSED.
*APPLIES ALL POSITIONING, ADAPTIVE, AND SAFETY EQUIPMENT PROPERLY.
*PROVIDES PRIVACY TO INDIVIDUALS.
*KNOWLEDGE OF ALL EMERGENCY PROCEDURES.
*COMPLETES BODY CHECKS FOR TEMPORARY ABSENCE OR DISCHARGE.
*FOLLOWS ALL INFECTION CONTROL POLICIES AND PROCEDURES.
*FEEDS OR ASSISTS INDIVIDUALS WITH EATING, FOLLOWING ALL DIETARY POLICIES
AND PROCEDURES.
*ACCOMPANIES INDIVIDUALS TO APPOINTMENTS OR HOSPITAL VISITS AS NEEDED.
*CONDUCTS ROOM CHECKS TO ENSURE INDIVIDUALS ARE CLEAN, DRY, COMFORTABLE
AND SAFE.
*IMPLEMENTS PROGRAMS, PROCEDURES AND BASELINES.
*COMPLETES ALL REQUIRED DOCUMENTATION INCLUDING BASELINES, MONTHLY TOTALS, CARE AND OBSERVATIONS AND INCIDENT REPORTS.
*IMPLEMENTS ACTIVITY SCHEDULES, PROVIDING MEANINGFUL ACTIVITIES FOR
INDIVIDUALS.
*ASSISTS CO-WORKERS AS NEEDED.
*COMMUNICATES INFORMATION AND/OR CONCERNS IN AN EFFECTIVE MANNER TO THE APPROPRIATE PERSONS.
*ENSURES SCHOOL/DAY PROGRAM EQUIPMENT IS READILY AVAILABLE.
*ASSISTS INDIVIDUALS IN DEVELOPING AND MAINTAINING ADL SKILLS.
*COMPLETES ALL CLEANING INCLUDING MAINTAINING INDIVIDUALS’ PERSONAL
ITEMS AND THEIR ROOMS.
*PRACTICES SAFETY TECHNIQUES TO PREVENT INJURY TO INDIVIDUALS SERVED, SELF
AND OTHER EMPLOYEES.
*COMMUNICATES WITH OTHERS ACCORDING TO HDC POLICIES, INFORMING THE SHIFT SUPERVISOR OF CONTACTS.
*FOLLOWS SHIFT CHANGE PROCEDURES.
*ATTENDS MEETINGS AND TRAININGS AS ASSIGNED.
*FOLLOWS HIPAA AND CONFIDENTIALITY POLICIES
*REPORTS SUSPECTED ABUSE.
*ADHERES TO THE CENTER’S CALL IN AND TIME OFF POLICY.
*MAINTAINS EXCELLENT ATTENDANCE AND PUNCTUALITY.
*FOLLOWS ALL HDC POLICIES AND PROCEDURES.
*UPHOLDS THE MISSION AND PHILOSOPHY OF THE CENTER.
*ENSURES ALL RESIDENTS ARE TREATED WITH DIGNITY AND RESPECT.
*PROVIDES NECESSARY SUPPORTS TO ALL RESIDENTS REGARDLESS OF HEALTH OR
BEHAVIORAL CONCERNS.

SOFTWARE AND PERFORM BASIC FUNCTIONS RELATED TO COMPUTER USE. (I.E.
KEYBOARDING, EMAIL, DATA ENTRY)
TRAINING AND EXPERIENCE:
PREVIOUS WORK WITH ADULTS OR CHILDREN WITH DISABILITIES OR NURSES AIDE EXPERIENCE PREFERRED.
