Description
PERSONAL INDEPENDENCE  & RESIDENTIAL CENTER INSTRUCTOR: Full-time position responsible for guiding, instructing and supervising a caseload of individuals with developmental disabilities. Openings for weekday 1st shift at PI Center 7:30 am-4:00 pm and every other weekend at Residential Center choice between 1st shift 6:00 am-2:30 pm or 2nd shift 2:00 pm-10:30pm. Instructors implement classroom instruction and activities for individual and group programming. Responsibilities also include daily personal care of participants. Will be required to have the ability to lift 50lbs. Must have valid driver’s license, insurance and reliable transportation. HS Diploma or GED required.

TITLE:    PERSONAL INDEPENDENCE SERVICES-RC INSTRUCTOR
RESPONSIBLE TO:    PERSONAL INDEPENDENCE SERVICES CASEMANAGER OR PERSONAL INDEPENDENCE SERVICES PROGRAM DIRECTOR AS ASSIGNED.
 
OBJECTIVE:    UNDER SUPERVISION OF PERSONAL INDEPENDENCE SERVICES CASEMANAGER OR PERSONAL INDEPENDENCE SERVICES PROGRAM DIRECTOR, TO GUIDE, INSTRUCT, AND SUPERVISE INDIVIDUALS, INCLUDING:
 
(A)       DAILY PREPARATION/PLANNING OF ACTIVITIES WITH EACH PARTICIPANT;
(B)       PREPARATION AND IMPLEMENTATION OF INDIVIDUAL PLANS; AND
(C)       DAILY PERFORMANCE EVALUATION ON EACH PARTICIPANT.
 
DUTIES:
  • NOTIFY PERSONAL INDEPENDENCE SERVICES CASEMANAGER OR
         INDEPENDENCE SERVICES PROGRAM DIRECTOR OF: PROGRAM NEEDS (SUPPLIES,
    ETC.); SPECIAL PARTICIPANT NEEDS (MEDICAL, INSTRUCTIONAL, ETC.);
    OUTSIDE CONTACTS (WITH PARENTS, RESIDENCES, ETC.).
  • PREPARATION OF ALL PARTICIPANT PROGRAM MATERIALS.
  • PURCHASE OF SUPPLIES WITH PROGRAM DIRECTOR'S APPROVAL.
  • DEVELOP AND IMPLEMENT FUNCTIONAL, AGE-APPROPRIATE ACTIVITIES FOR
    CLASSES.
  • MEET WITH PROGRAM DIRECTOR WEEKLY AND AS NEEDED.
  • OBTAIN AND MAINTAIN KNOWLEDGE OF CURRENT TRENDS IN INTELLECTUAL/
    DEVELOPMENTAL DISABILITY, CARF STANDARDS, ICF/ID AND HCBS
    REGULATIONS.
  • ATTEND ALL STAFF MEETINGS.
  • SUBMIT ALL CORRESPONDENCE AND PLANS FOR PROGRAM DIRECTOR APPROVAL.
  • ADHERE TO SAFETY PROCEDURES/PRACTICES.
  • ASSIST PARTICIPANTS WITH PERSONAL CARE.
  • ASSIST PARTICIPANTS WITH EATING.
  • ASSIST PARTICIPANTS WITH PHYSICAL THERAPY.
  • PROVIDE LEISURE ACTIVITIES FOR INDIVIDUALS WAITING FOR VANS TO ARRIVE.
  • STOCK CHANGING ROOMS AND ENSURE LAUNDRY IS DONE.
  • MAINTAIN EXCELLENT ATTENDANCE AND PUNCTUALITY TO ENSURE ADEQUATE STAFFING LEVELS TO MEET THE NEEDS OF PERSONAL INDEPENDENCE SERVICES PARTICIPANTS
                       *     ADHERE TO THE CENTER’S ATTENDANCE AND CALL IN PROCEDURES.   
                       *     COMPLETES ASSIGNED NURSING DUTIES.
                       *     KEEPS INDIVIDUALS CLEAN AND APPROPRIATELY DRESSED.
                       *     APPLIES ALL POSITIONING, ADAPTIVE, AND SAFETY EQUIPMENT PROPERLY.
                       *     PROVIDES PRIVACY TO INDIVIDUALS.
                       *     KNOWLEDGE OF ALL EMERGENCY PROCEDURES.              
  •                    *     FOLLOWS ALL INFECTION CONTROL POLICIES AND PROCEDURES.                                                       
  •                    *     STAYS ABREAST OF INDIVIDUALS’ CONDITION, NEEDS AND WHEREABOUTS.
                       *     COMPLETES ALL RC DOCUMENTATION. 
                       *     IMPLEMENTS ACTIVITY SCHEDULES, PROVIDING MEANINGFUL ACTIVITIES FOR INDIVIDUALS.                                                    
                       *     COMMUNICATES INFORMATION AND/OR CONCERNS TO THE APPROPRIATE
                                PERSONS.                                                                                                                                                        
  •                    *     REPORTS SUSPECTED ABUSE.
  •                    *     FOLLOWS ALL HDC POLICIES AND PROCEDURES.
                       *     MAINTAINS EXCELLENT ATTENDANCE AND PUNCTUALITY.
                              *     UPHOLDS THE MISSION AND PHILOSOPHY OF THE CENTER
                              *     ENSURES ALL RESIDENTS ARE TREATED WITH DIGNITY AND RESPECT.
                              *     PROVIDES NECESSARY SUPPORTS TO ALL RESIDENTS REGARDLESS OF HEALTH OR BEHAVIORAL CONCERNS.                 
  OTHER DUTIES AS ASSIGNED.

 WILLINGNESS TO SEEK OUT ADDITIONAL DUTIES.
ABILITY TO MEET EMPLOYMENT REQUIREMENTS (I.E. HEALTH SCREENING, TB TEST).
ABILITY TO USE COMPUTER TO NAVIGATE AND ENTER INFORMATION INTO WEB-BASED SOFTWARE
AND PERFORM BASIC FUNCTIONS RELATED TO COMPUTER USE (I.E. KEYBOARDING, EMAIL, DATA ENTRY)
 
TRAINING AND EXPERIENCE:
HIGH SCHOOL DIPLOMA OR EQUIVALENT GED, TWO YEARS RELATED WORK
EXPERIENCE, DEGREE PREFERRED, BUT PROVEN COMPETENCE IN
RELATED JOB EXPERIENCE, WRITING SKILLS AND ADAPTABILITY WOULD
QUALIFY.
