Full-time professional position is responsible for coordinating and monitoring services to assigned CRS participants. This includes development of service plans, implementation of services and necessary documentation and paperwork. Duties also include scheduling and supervision of staff and rotating on-call responsibility.
Bachelor’s degree in Human Services required; experience working in ID/DD setting preferred. Ability to use basic computer programs (Microsoft Office suite) to complete electronic documentation and basic computer functions.

TITLE: RESIDENTIAL CASEMANAGER

RESPONSIBLE TO: COMMUNITY RESIDENTIAL SERVICES PROGRAM DIRECTOR OR ASSISTANT

PROGRAM DIRECTOR

 

KIND OF WORK:

 

OBJECTIVE: COORDINATE AND MONITOR SERVICES TO ASSIGNED CRS PARTICIPANTS

INCLUDING DEVELOPMENT OF SERVICE PLAN, IMPLEMENTATION OF SERVICES AND

NECESSARY DOCUMENTATION. SUPERVISE ASSIGNED SERVICE STAFF.

 

DUTIES:

  • WITH OTHER RESIDENTIAL STAFF AND INDIVIDUALS, EVALUATE EACH INDIVIDUAL'S FUNCTIONING AND ASSESS HIS/HER SERVICE NEEDS.

  • WORK WITH DIRECT SERVICES STAFF AND PARTICIPANT TO FORMULATE GOALS AND OBJECTIVES FOR EACH PERSON, TO ACCURATELY DOCUMENT AND ASSESS

    PROGRESS TOWARD SUCH GOALS AND TO ASSESS INDIVIDUAL NEEDS OF

    PARTICIPANTS.

  • PROVIDE APPROPRIATE CASEWORK AND COUNSELING SERVICES TO INDIVIDUALS, INDIVIDUALLY AND/OR IN SMALL GROUPS, AS APPROPRIATE.

  • MAINTAIN ACCURATE COMMUNICATION WITH FAMILIES OF INDIVIDUALS, AND

    PROVIDE COUNSELING AS APPROPRIATE.

  • MAINTAIN ACCURATE PROFESSIONAL RECORDS ON EACH INDIVIDUAL TO DOCUMENT

    PLANNING, PROGRESS AND REASSESSMENT BY RESIDENTIAL STAFF, AS WELL AS

    TO RECORD ESSENTIAL DATA REGARDING INDIVIDUAL’S ACTIVITIES, MEDICAL

    NEEDS AND TREATMENT, AND OTHER SIGNIFICANT EVENTS.

  • DEVELOP AND MAINTAIN GOOD CHANNELS OF COMMUNICATION AMONG RESIDENTIAL STAFF, AND BETWEEN RESIDENTIAL STAFF AND OTHER HDC PROGRAM PERSONNEL INVOLVED WITH PARTICIPANTS. THE GOAL IS COORDINATED AND CONSISTENT PLANNING AND PROGRAMMING FOR EACH INDIVIDUAL.

  • PROVIDE INFORMATION TO INDIVIDUALS, GROUPS, AGENCIES, AND PARTICIPANTS

    REGARDING THE CENTER’S PROGRAMS.

  • ACQUIRE WORKING KNOWLEDGE OF LAWS, REGULATIONS, AND POLICY UNDER WHICH PROGRAM IS OPERATED AS REQUIRED TO DISCUSS SUCH MATTERS WITH PARTICIPANTS, FAMILIES, AND OTHER AGENCIES AND TO ENSURE COMPLIANCE WITH THE INDIVIDUAL PROGRAMS.

  • ASSIST COMMUNITY RESIDENTIAL SERVICES PROGRAM DIRECTOR IN PLANNING AND

    DEVELOPING COMMUNITY RESIDENTIAL SERVICES PROGRAM.

  • PROVIDE TRAINING AND DAY-TO-DAY GUIDANCE OF DIRECT SERVICE STAFF.

  • ASSIST IN TRAINING AND SUPERVISION OF VOLUNTEERS WORKING WITH

    PARTICIPANTS.

  • RELATE TO OTHER SOCIAL SERVICE, HEALTH, AND EDUCATIONAL AGENCIES IN THE COMMUNITY ON BEHALF OF PARTICIPANTS.

  • SUPERVISE AND COORDINATE THE WORK OF ASSIGNED RESIDENTIAL STAFF.

  • RELATE CONTACT WITH OUTSIDE AGENCIES/INDIVIDUALS IMMEDIATELY TO SUPERVISOR.

  • ACT AS A MEDICATION MANAGER AS NEEDED.

  • MAINTAIN EXCELLENT ATTENDANCE AND PUNCTUALITY TO ENSURE PARTICIPANT

    NEEDS ARE MET. REGULAR AND ON SITE ATTENDANCE REQUIRED.

  • RESPONSIBLE FOR ON-CALL DUTIES.

    OTHER DUTIES AS ASSIGNED.

     

    QUALIFICATIONS:

     

    KNOWLEDGE AND ABILITIES:

    KNOWLEDGE OF THE SPECIAL PROBLEMS OF PERSONS WITH DISABILITIES AND STRONG

    COMMITMENT AND INTEREST IN BRINGING ABOUT CHANGE.

    KNOWLEDGE OF COMMUNITY SOCIAL, EDUCATION AND HEALTH AGENCIES AND THE

    SERVICES THEY PROVIDE.

    ABILITY TO EXPRESS ONESELF WELL, BOTH VERBALLY AND IN WRITTEN FORM.

    ABILITY TO LIFT FROM FLOOR/CARRY/TRANSFER INDIVIDUALS AND ADAPTIVE

    EQUIPMENT IN EXCESS OF 50 POUNDS.

    ABILITY TO USE BASIC COMPUTER PROGRAMS (MICROSOFT OFFICE SUITE) TO COMPLETE ELECTRONIC DOCUMENTATION AND PERFORM BASIC FUNCTIONS RELATED TO COMPUTER USE. (I.E. KEYBOARDING, EMAIL)

    POSSESS A CURRENT AND VALID DRIVER’S LICENSE AND MAINTAIN A GOOD

    DRIVING RECORD. MUST NOT BE DENIED COVERAGE BY HDC’S INSURANCE

    CARRIER.

    NO CURRENT RECORD OF HABITUAL ALCOHOL INTOXICATION OR ADDICTION TO THE

    USE OF DRUGS.

    ABILITY TO CARRY/ESCORT INDIVIDUALS IN EXCESS OF 50 POUNDS AND UTILIZE CPI

    TECHNIQUES DURING BEHAVIOR PROBLEMS.

    ABILITY TO USE BASIC COMPUTER PROGRAMS (MICROSOFT OFFICE SUITE) TO

    COMPLETE ELECTRONIC DOCUMENTATION AND PERFORM BASIC FUNCTIONS

    RELATED TO COMPUTER USE. (I.E. KEYBOARDING, EMAIL, DATA ENTRY)

ABILITY TO USE COMPUTER TO NAVIGATE AND ENTER INFORMATION INTO WEB-BASED

SOFTWARE

 

 

TRAINING AND EXPERIENCE:

BACHELOR’S DEGREE IN HUMAN SERVICES, SOCIAL WORK, EDUCATION OR A RELATED

FIELD AND A MINIMUM OF ONE YEAR EXPERIENCE IN SOCIAL SERVICES.

EXPERIENCE IN A SETTING WITH PEOPLE WITH INTELLECTUAL DISABILITIES IS

PREFERRED.
