Description

 

This full-time 1st shift M-F, 7:30 AM-4:00 PM, position is responsible for guiding, supervising and implementing classroom instruction and activities for individual and group programming. Will be required to have the ability to lift 50lbs. Previous experience with ID/DD preferred. Preferred candidate will have valid driver’s license, insurance and reliable transportation. Ability to use basic computer programs (Microsoft Office suite) to complete electronic documentation and basic computer functions. HS diploma or GED required.

$10.00/hr.

Title: Personal Independence Services Instructor

Responsible to: Personal Independence Services Casemanager or Personal Independence Services Program Director as assigned.

 

Objective: Under supervision of Personal Independence Services Casemanager or Personal Independence Services Program Director, to guide, instruct, and supervise individuals, including:

  1. Daily preparation/planning of activities with each participant;
  2. Preparation and implementation of individual plans
  3. Daily performance evaluation on each participant.

 

Duties:

 

  • Treat participants/residents with dignity and respect.

  • Notify Personal Independence Services Casemanager or Personal Independence Services Program Director of: Program needs (supplies, etc.); special participant needs (medical, instructional, etc.); outside contacts (with parents, residences, etc.)

  • Preparation of all participant program materials.

  • Purchase of supplies with Program Director’s approval.

  • Preparation of narrative reports and lesson plans.

  • Develop and implement functional, age-appropriate activities for classes.

  • Meet with Program Director weekly and as needed.

  • Obtain and maintain knowledge of current trends in intellectual/developmental disability, Carf standards, ICF/ID and HCBS regulations.

  • Attend all staff meetings.

  • Submit all correspondence and plans for Program Director approval.

  • Adhere to safety procedures/practices.

  • Assist participants with personal care.

  • Assist participants with eating.

  • Assist participants with physical therapy.

  • Provide leisure activities for individuals waiting for vans to arrive.

  • Stock changing rooms and ensure laundry is done.

  • Inform supervisor at least two weeks prior to using vacation or personal time whenever possible.

  • Follows HIPAA and confidentiality policies.

    Ability to carry/escort individuals in excess of 50 pounds and utilize PBS techniques during behavior problems

    Ability to work as part of a team

    Willingness to seek out additional duties

    1. to use basic computer programs (Microsoft office suite) to complete electronic documentation and performs basic functions related to computer use (i.e. typing, email).

    Ability to meet employment requirements (i.e. health screening, TB Test)

    Ability to use computer to navigate and enter information into web-based software and perform basic functions related to computer use (i.e. keyboarding, email, data entry)

    Ability to understand and follow the policies and procedures of HDC

     

    Training and Experience:

     

    High school diploma or equivalent GED, two years related work experience, degree preferred, but proven competence in related job experience, writing skills and adaptability would qualify.
