Empowering Abilities is looking for a Casemanager that is able to be a role model and leader for employees and the individuals we serve. In this role you will advocate for participants and create objectives that foster independence. You will be working within our agency and with outside agencies to assist our participants to succeed, to achieve, to grow and be happy. You will be supervising direct care staff, making sure they have the necessary resources to be successful. You will also take a hands on role with participants when needed.

This position works Monday-Friday from 7:30 am-4:00 pm, with on call duties every other month. Empowering Abilities offers great benefits including health and vacation to name a few. Qualified candidate will need a Bachelor’s degree and a valid drivers license.

Title: Daily Discovery Casemanager
Responsible to: Chief Programming Officer and Employment Services or Assistant Program Director

Objective: To carry out intake procedures, rehabilitation and habilitation counseling of participants receiving Prevocational, Community Employment, and/or Day Habilitation services, organize and maintain services, and in cooperation with other staff, formulate and follow through on individual plans for participants.
