JOB COACH: Part-Time position , up to 29 hours, to assist adults with disabilities to be successfully employed in the community. Assist with job development, on-site training, and transportation. Schedule is flexible, but will require some morning or evening hours, and occasional holidays and weekends. Reliable transportation, valid driver’s license, current auto insurance and HS Diploma or GED required. Will be required to have the ability to lift 50lbs.

TITLE: JOB COACH
RESPONSIBLE TO: EMPLOYMENT SERVICES CASEMANAGER
KIND OF WORK:
OBJECTIVE: SERVE AS ON-SITE COORDINATOR OF SUPPORTED EMPLOYMENT SERVICES.
SUPERVISE AND IMPLEMENT SHORT AND LONG-TERM GOALS AND OBJECTIVES FOR
COMMUNITY EMPLOYMENT AND PARTICIPANTS.
DUTIES:
* MAINTAIN EXCELLENT ATTENDANCE AND PUNCTUALITY TO ENSURE PARTICIPANTS
MEET ATTENDANCE AND PUNCTUALITY EXPECTATIONS AT THEIR INDIVIDUAL JOB SITES.
* AID IN THE DEVELOPMENT OF APPROPRIATE WORK SITES FOR PARTICIPANTS.
* PLAN, DEVELOP AND IMPLEMENT TRAINING AND SUPERVISION FOR
PARTICIPANTS.
* SUPERVISE AND/OR COMPLETE ASSIGNED SUPPORTED EMPLOYMENT
CONTRACTS AND JOB TASKS AT INDIVIDUAL WORK SITES.
* WORK A FLEXIBLE AND ROTATING SCHEDULE INCLUDING NIGHTS, WEEKENDS, AND
HOLIDAYS.
* NOTIFIES SUPERVISOR IMMEDIATELY WHEN INITIAL, SPECIFIC TRAINING IS NEEDED
PRIOR TO WORKING AN ASSIGNMENT.
* MAINTAIN ON-GOING AWARENESS OF INDIVIDUAL’S PROGRESS BY MEANS OF
OBSERVATION AND RECORDKEEPING, AND REPORT WEEKLY TO
SUPERVISOR.

COMPLETE AND MAINTAIN THE NECESSARY EMPLOYMENT SERVICES TRAINING AS REQUIRED BY FUNDING AND AUDITING ENTITIES.
* REPRESENT THE AGENCY AND MAINTAIN THE ROLE OF LIAISON WITH A WIDE VARIETY OF SUPPORT GROUPS, EMPLOYERS AND INDIVIDUALS.
* PROVIDE INDIVIDUAL, GROUP, AND/OR FAMILY COUNSELING AS
REQUIRED.
* PARTICIPATE IN MEETINGS WITH PARTICIPANTS AND INTERESTED PARTIES
FOR PURPOSE OF REVIEWING PROGRAM EFFECTIVENESS, PROGRESS
AND ANY OTHER PROCEDURAL MATTER REQUIRED TO ENSURE
SUCCESSFUL EMPLOYMENT.
PERFORM OTHER DUTIES AS ASSIGNED
QUALIFICATIONS:
KNOWLEDGE AND ABILITIES:
ABILITY TO SUPERVISE, MOTIVATE, INSTRUCT, AND EVALUATE PARTICIPANTS.
ABILITY TO COMMUNICATE EFFECTIVELY.
KNOWLEDGE OF GOOD SAFETY PRACTICES AND METHODS.
KNOWLEDGE OF GOALS, METHODS, AND PRACTICES IN INDUSTRY.
ABILITY TO USE BASIC COMPUTER PROGRAMS (MICROSOFT OFFICE SUITE) TO COMPLETE ELECTRONIC DOCUMENTATION AND PERFORM BASIC FUNCTIONS RELATED TO COMPUTER USE. (I.E. KEYBOARDING, EMAIL, DATA ENTRY)
ABILITY TO USE COMPUTER TO NAVIGATE AND ENTER INFORMATION INTO WEB-BASED SOFTWARE
ABILITY TO LIFT/CARRY/TRANSFER INDIVIDUALS AND EQUIPMENT IN EXCESS OF
50 POUNDS USING PROPER LIFTING TECHNIQUES.
